Gift aid declaration

If you are a UK taxpayer, the value of your donation can increase by 25% under the Gift Aid scheme - at no additional cost to you. Please tick the box below to join the Gift Aid scheme.

I am a UK taxpayer. Please treat all donations I make or have made to DKMS UK for the past four years as Gift Aid donations until I notify you otherwise. I understand that I must have paid or will pay an amount of Income Tax and/or Capital Gains Tax in each year, that is at least equal to the tax that Charities & Community Amateur Sports Clubs (CASCs) I donate to will reclaim on my gifts. I understand that other taxes such as VAT and Council Tax do not qualify and that DKMS UK will reclaim 25p of tax on every £1 that I give.

The Direct Debit Guarantee

- I have read the following Direct Debit Guarantee
- This Guarantee is offered by all Banks and Building Societies that accept instructions to pay direct debits.
- If there are any changes to the amount, date or frequency of your direct debit PSL re DKMS Foundation will notify you 5 days in advance of your account being debited or as otherwise agreed. If you request PSL re DKMS Foundation to collect a payment, confirmation of the amount and date will be given to you at the time of the request.
- If an error is made in the payment of your direct debit, by PSL re DKMS Foundation or your Bank or Building Society, you are entitled to a full and immediate refund of the amount paid from your bank or building society.
- If you receive a refund you are not entitled to, you must pay it back when PSL re DKMS Foundation asks you to
- You can cancel a Direct Debit at any time by simply contacting your bank or Building Society. Written confirmation may be required. Please also notify us.
